Serve as the primary contact for complex-care clients, coordinate care across providers, develop and update care plans, connect clients to social and medical services, document in EMR/CRM, perform outreach and engagement, and report on program compliance to promote wellness and access to services.

Provide clinical oversight of ECM Lead Care Managers (10–15 direct reports), supervise creation and execution of individualized care plans, escalate and manage complex/behavioral health cases, coach staff to meet KPIs, collaborate with PCPs and behavioral health providers, implement process improvements, and provide direct care as needed. Travel up to 20–25% for training and meetings.
