To help share our mission and expand our community impact, Vitra Health is seeking a Community Liaison II who is passionate about community engagement and helping people access the care and resources they deserve. 

The Opportunity 

The Community Liaison role is responsible for supporting sales and leading generation efforts across GAFC, AFC, VNA, and ADH program development. This includes developing, implementing, and executing a business plan within the assigned territory, as well as generating new partnerships and client leads within the community. The individual will assist in initiating the enrollment process for eligible members in the field and support the intake process for prospective members. Additionally, the role focuses on building brand awareness of Vitra Health within local communities. 

This role supports the growth and development of the GAFC, AFC, VNA, and ADH programs by assisting with sales initiatives and lead generation efforts throughout the assigned territory. Responsibilities include developing, implementing, and executing strategic business plans, generating new community partnerships and referral sources, and identifying potential client leads. The position also supports the enrollment and intake processes for eligible and prospective members in the field, ensuring a smooth transition into services. Additionally, the role is responsible for promoting Vitra Health's services and increasing brand awareness within local communities to drive program growth and engagement. 

Key Responsibilities  

  • Enroll eligible members into GAFC, AFC, VNA, and ADH programs in alignment with clinical and insurance eligibility requirements 
  • Develop and maintain relationships with prospective clients through community engagement, events, and outreach activities 
  • Meet or exceed established performance and referral goals (see performance metrics sheet) 
  • Generate, track, and follow up on business development leads in assigned territory 
  • Build and maintain relationships with healthcare providers, day habilitation programs, community health centers, housing authorities, and community-based organizations 
  • Establish and maintain key partnerships with residential communities and local agencies 
  • Conduct in-field outreach to increase awareness and drive membership growth 
  • Develop and execute weekly, monthly, and quarterly outreach plans aligned with business objectives 
  • Provide translation services for current and prospective clients as needed 
  • Support prospective members through intake and enrollment processes, including assistance with government benefits and applications 
  • Conduct on-site visits with clients, referral sources, and community partners 
  • Participate in outreach planning, strategy development, and implementation initiatives 
  • Leverage existing relationships and partnerships to expand referral networks 
  • Assist in the development and execution of new program outreach and partnership strategies 
  • Maintain accurate and timely documentation in CRM and intake systems 
  • Provide regular reporting and data insights on outreach and sales activity 
  • Attend required status, planning, and strategy meetings 
  • Assist in caregiver recruitment efforts within assigned territory 
  • Ensure 100% compliance with all required certifications, training, licensing, policy acknowledgements, and continuing education requirements 
  • Meet or exceed monthly outreach, referral, and enrollment goals through field-based activities including visits to facilities, networking events, primary care offices, housing communities, and community events 
  • Perform other duties as assigned by management 

Essential Functions 

  • Demonstrate comprehensive knowledge of all Vitra Health programs and clearly communicate the Vitra Health value proposition compared to competitors 
  • Develop and maintain a strategic outreach plan focused on partnership development and membership growth 
  • Execute consistent field-based outreach to generate new referrals and partnerships 
  • Maintain accurate CRM documentation and reporting of all outreach and sales activities 
  • Support end-to-end client engagement from outreach through enrollment 
  • Perform additional responsibilities as needed to support business objectives 

What we are looking for:   

  • 3–5 years of sales experience with a proven track record of meeting or exceeding performance goals 
  • Demonstrated ability to build and maintain strong professional relationships within community-based networks 
  • Ability to work independently and perform 100% field-based work 
  • Experience using Google Suite, Microsoft Office Suite, HubSpot, and Salesforce preferred 
  • Bilingual in English and a second language preferred 
  • Willingness to work evenings and weekends as needed 
  • Prior experience in home health care or healthcare-related field preferred 

Work Environment and Physical Requirements 

  • Office-based work in a clean well-lit environment with fluctuating temperatures near others. 
  • Client-facing field staff and community liaisons require frequent travel to client homes or community settings; ability to drive safely; work in client homes may involve varying temperatures, odors, allergens, pets, and other environmental factors. 
  • Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse. 
  • Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds; requires walking and standing; requires frequent sitting more than 75% of the workday; requires the ability to negotiate stairs; requires visual acuity and manual dexterity to operate equipment.
