Job Description:
Reporting to the Director, Health Services Administration, the Senior Case Manager works in consultation and collaboration with internal and external partners to assist students in accessing supports for health-related concerns and navigating barriers to success. This position provides 1:1 consultation with students, and supports essential functions related to interventions with students of concern.
In addition to carrying a caseload, the Senior Case Manager supervises a team of three Case Managers.
Case management services will include coordination with various units in and outside of Student Affairs as well as external resources (e.g. social service agencies, insurance companies, health care providers).
While this is not a clinical position, sensitivity, judgment and respect for confidentiality is required. This position will facilitate support for students/parents/family members during both urgent and non-urgent situations, many of which involve mental and physical challenges. The Senior Case Manager helps ensure appropriate care and follow-up for students in need of additional resources, helping to coordinate a comprehensive plan of care with appropriate colleagues across multiple units of the College.
MINIMUM JOB QUALIFICATIONS: Bachelor’s level social work, counseling, education, health sciences, or equivalent experience. Masters preferred. Experience working in a college, university or healthcare environment is preferred. Specific skills and experience in health services relevant to late adolescents/young adults, counseling services, and/or student affairs in a higher education environment preferable.
ESSENTIAL FUNCTIONS/PRIMARY DUTIES AND RESPONSIBILITIES:

Provides on-going training, professional development opportunities and onboarding of new staff aligned with best practices.

Conducts annual performance evaluations for direct reports

Provides weekly 1:1 supervision to direct reports

Assists in developing policies and procedures aligned with professional associations (e.g., HECMA, NABITA) and field research.

Assists in the coordination of care during and after student hospitalizations.

Maintains a caseload of students with chronic psychosocial challenges, with a preference for in-person meetings.

Represents department at college wide events including but not limited to orientation, staff and faculty development, Health and Wellness events, ongoing campus training.

Assists in the development and review of social media content.

Coordinates with Valencia for consistency of approaches to offer case management services.

Curates and maintains a comprehensive list of off-campus public and private resources. Establishes working relationships with these resources, agencies, organizations.

Review and provide outreach to student concerns submitted as health and wellness early alerts; engage appropriate on and off campus parties in the response where necessary, via walk-in and scheduled appointments, assist students in accessing off-campus health care and community resources , using health insurance, and assisting students in outreach to providers as necessary; develop and maintain relevant documentation for those cases.

Under the direction of the Director, Health Services Administration, collaborates with members of CARES to triage and respond to community generated reports regarding enrolled students-, coordinate follow up plans as needed, and maintain relevant documentation for the students-of-concern CARES committee.

Communicate with parents/family members, faculty, and other necessary partners to assist with coordinated plans and management of student concerns.

Support counseling services in coordinating off-campus referrals.

Provides and/or coordinates front-line coverage as needed.

Other duties as assigned.
WORK ENVIRONMENT: Supervisors are responsible for maintaining an environment that is free from discrimination, intimidation, coercion or harassment, including sexual harassment. Work is performed indoors and primarily in an office setting. The noise level is usually low to moderate. While performing the duties of this position, the employee may be required to travel.
