Schedule: 5:30pm - 2:00pm, Monday- Friday
Pay rate: $22.90-$26.51

The Recovery Support Navigator is responsible for:
- Providing quality services and case management to assigned patients in accordance with the Recovery Navigator model.
- Performing intake assessments on all new patients to the program and completes all associated documentation within required time frames.
- Working with the patient to develop and implementing an individualized plan with goals and objectives based on identified needs.
- Assisting patients with scheduling and coordinating medical and behavioral health appointments.
- Coordinating care and the plan with existing providers including but not limited to behavioral health, primary care and Community Partner.
Qualifications:
- Bachelor’s Degree in human services related field or discipline
- Or LADC
- Or has a recognized certification and/or licensure for the target population to work with
- Or sufficient experience and training in counseling and meets regulatory requirements
