- Help members set goals and develop a care plan to achieve those goals
- Proactively engage the members to manage their own health and healthcare
- As needed, help the member engage with mental health and substance use treatment
- Utilize both company and community-based resources to establish a safe and effective case management plan for members
- Collaborate with patients, family, and healthcare providers
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services

Required Qualifications:
- High School Diploma/GED
- 1+ years of computer proficiency experience including MS Word, Excel and Outlook
- 6+ months of field-based experience of community advocacy
- Solid knowledge of regulatory requirements, competent in case management business as well as clinical arenas Demonstrated solid technology skills and excellent interpersonal skills
- Demonstrated ability to negiotiate and arbitrate without difficulty
- Demonstrates activities for follow-through
- Ability to travel locally within Las Vegas Metro and complete documents for Reimbursed mileage
- Resides within the local community
- Unrestricted Driver's License
Preferred Qualification:
- Experience in case management, community outreach or volunteer activities with interaction with the public
