Community Care Cooperative (C3)

Community Care Cooperative (C3) is a nonprofit, FQHC-governed Accountable Care Organization that brings federally qualified health centers together to improve community health. It supports integrated, coordinated primary care through Medicaid and Medicare ACO participation, provider collaboration, population-health and data capabilities, pharmacy and technology services, telehealth integration, consulting, and shared services. Its mission is to improve health and wellness, strengthen health centers, and advance equitable care for underserved communities.
