Position: Stroke Nurse Coordinator

Department: Nursing Administration

Schedule: Full Time (40 Hours)

Position Summary:

The Stroke Nurse is the clinical and administrative nurse lead for the Stroke Program at Boston Medical Center- South The Stroke Nurse Coordinator reports directly to Chief Nursing Officer and a dotted line to the Sr. Director of Quality and Safety. The position collaborates with hospital clinical and administrative leadership as well as professional organizations to facilitate the development of the Stroke Service at Boston Medical Center-South and leads or participates in meetings as appropriate to assure effective communication and provides expertise in the areas of stroke specialization. The Stroke Nurse Coordinator shall develop and promote a standard of care consistent with service excellence at the hospital and outpatient level in accordance with the goals established in the guidelines of DPH, Joint Commission, and BMC policies and procedures. The Stroke Nurse works closely with the broad range of healthcare providers, administrators, and patients and families.

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Coordinate clinical stroke quality improvement program, including data collection, monitoring and integrating several staff from multiple non-stroke related disciplines.
  • Coordinate all aspects of Stroke Quality Improvement, both regulatory and research related.
  • Lead and collaborate on neurology education around stroke for the institution and the system. Serves as a leader in patient stroke education throughout the hospital, working with bedside nurses, therapists (PT, OT, SLP), and the Neurology team to provide stroke education.
  • Responsible for outcomes measurement and performance improvement for selected population.
  • Coordinates with Quality and Safety as well as the Chief Medical Officer and others to evaluate new American Stroke Association and Massachusetts Public Health guidelines for inclusion in department monitoring processes.
  • Participate in local, regional and national committees and programs, as appropriate.
  • Coordination, maintenance, and growth of the Stroke Program in collaboration with the Director of the SEMC Stroke Service through professional relationships with interdisciplinary staff, intensivists, hospitalists, and emergency physicians.
  • Conduct monthly Stroke Program meetings for the evaluation of outcomes, policy and protocol development, and review of current best practices.
  • Provide leadership to enhance current Stroke Program through networking and ongoing education.
  • Coordination of process improvement activities which include assignment to various departments for required follow up to improve performance outcomes.
  • Responsible to track, report, and submit hospital Stroke data to Quality Department and national benchmarking agency(s).
  • Consult with healthcare team and patients during hospitalization (including discharge instructions and post discharge follow-up) and following hospitalization to monitor adherence to plan of care. Development and collaboration on patient teaching materials.
  • Responsible for staff development regarding care of the Stroke patient and development of related materials.
  • Participate in clinical care in inpatient and outpatient settings.
  • Accountable for maintaining the confidentiality and security of all hospital-related, medical staff-related, and patient-related data and information according to HIPAA guidelines and medical center policies and procedures
  • Collects and analyzes data on stroke program outcomes to identity areas for improvement and ensures the program adheres to regulatory requirements and certification standards.
  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment
  •  Performs other similar and related duties as required or directed by the chair of the department
  • Participates in community outreach and education initiatives in collaboration with the Chair of Neurology, Director of the Stroke Program
  • Facilitate outreach with Emergency Medical services, community hospitals, EMS and other medical providers outside of BMC to develop and implement strategies that aim to improve the overall stroke system of care
  • Performs other similar and related duties as required or directed by the chair of the department

JOB REQUIREMENTS

REQUIRED EDUCATION AND EXPERIENCE:

  • Bachelor's degree in Nursing and three to five years of Acute Care, Critical Care, and/or ED experience with related pathophysiology required.

PREFERRED EDUCATION AND EXPERIENCE:

  • Master's degree and Three to five years of education or program development experience preferred. Previous experience in case management, stroke care and knowledge of community resources, data collection and analysis is desired.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

  • Current MA RN license

CERTIFICATES, LICENSES, REGISTRATIONS PREFERRED:

  • Stroke Program Coordinator Certificate or ability to obtain within 6 months of hire

KNOWLEDGE, SKILLS & ABILITIES (KSAs):

  • Effective interpersonal communication skills
  • Proficient in Microsoft office

Compensation Range:

$83,000.00- $120,500.00

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
