Job Purpose:
- The SRO Case Manager at Merrimack Valley YMCA is responsible for providing comprehensive case management services to Single Room Occupancy (SRO) residents. The role involves assessing resident needs, developing individualized service plans, and facilitating access to community resources to support residents in achieving stability and self-sufficiency.
Key Responsibilities:
- Conduct initial assessments and ongoing evaluations of residents' needs, strengths, and barriers to develop personalized service plans.
- Collaborate with residents to set achievable goals and monitor progress towards self-sufficiency and stability.
- Facilitate access to community resources, including healthcare, employment services, educational opportunities, and housing support.
- Advocate for residents' needs within the organization and with external service providers.
- Maintain accurate and up-to-date case records, documenting interactions, progress, and outcomes.
- Coordinate with other YMCA departments and external agencies to ensure comprehensive support for residents.
- Provide crisis intervention and support as needed, including conflict resolution and emergency assistance.
- Organize and lead workshops or group activities aimed at enhancing residents' skills and knowledge.
- Participate in regular team meetings and training sessions to stay informed about best practices and new resources.
- Ensure compliance with organizational policies and procedures, as well as local, state, and federal regulations.
Required Education:
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
Required Experience:
- Minimum of 2 years of experience in case management or social services.
- Experience working with diverse populations, including individuals experiencing homelessness or housing instability.
- Proven track record of developing and implementing individualized service plans.
Required Skills and Abilities:
- Strong understanding of community resources and referral processes.
- Excellent communication and interpersonal skills to effectively interact with clients and stakeholders.
- Ability to work independently and collaboratively within a team environment.
- Proficient in maintaining accurate and detailed case records.
- Demonstrated problem-solving skills and ability to handle crisis situations.
- Familiarity with relevant laws and regulations pertaining to housing and social services.
