The BSW Intern provides direct support to members by assisting with resource navigation, outreach, intake, and care coordination tasks. Interns help guide members through social determinants of health needs, document interactions in the Impact Connections platform, and collaborate with the team to ensure members receive timely, compassionate support. This role offers hands-on experience in case management, community resource identification, and member engagement within a tech-enabled social care model.

Key Responsibilities:

  • Conduct outreach to members and assist with scheduling, follow-up, and check-ins.
  • Support members in accessing community resources and navigating services.
  • Complete intakes, care plan updates, and documentation 
  • Assist with resource research and maintaining updated resource lists.
  • Participate in team meetings, supervision, and member case reviews.
  • Maintain professionalism, confidentiality, and trauma-informed communication.


Requirements
  • Must be currently enrolled in a Bachelor of Social Work (BSW) program and eligible for a field placement or internship.
  • Ability to commit to 8–16 hours per week (or program-required hours).
  • Strong written and verbal communication skills.
  • Demonstrated interest in case management, community resources, and Social Determinants of Health (SDOH).
  • Ability to maintain confidentiality and use trauma-informed, culturally responsive communication.
  • Comfort using digital tools, including Google Workspace and basic CRM/case management platforms (training provided).
  • Strong organizational skills and ability to manage multiple tasks and deadlines.
  • Ability to work independently, take initiative, and seek supervision when needed.
  • Reliable access to a computer and stable internet connection for virtual work.
  • Professionalism, empathy, and alignment with Impact Connections’ mission and values.
