JOB SUMMARY:

Under general supervision by the Director of Quality and Safety, the Risk Manager assists the Director with the development and implementation of the Hospital-wide Quality and Safety Plan consistent with board approved plans and regulatory and standard setting agencies.

The Risk Manager will also share risk data with BMC main campus and participate in conferences with BMC main campus risk department. Investigate every incident report filed and work with involved parties, and Patient Safety Triage Team to identify and address opportunities for improvement.  The Risk Manager creates presentations, organizes meetings, and takes notes. This role also files mandated reporting to the DPH and CMS.

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Provides data to the Quality and Patient Safety Committee, Patient Safety Triage Team, Performance Improvement Task Forces, and other related committees. Attend committee/team/task force meetings and create and distribute minutes. Present incident reports and mandated reporting that has occurred.

  • Attend departmental/hospital meetings where measurement and evaluation functions and quality management activities are performed, as agreed upon by the Director of Quality and Safety.

  • Attends Performance Improvement Task Force meetings and Collaborative Group Practice meetings where performance improvement activities are conducted

  • Assist with the annual evaluation of the Q & S Plan and Program, on behalf of the Q S Committees and analyze risk reporting.

  • Assists with follow-up of patient safety and risk issues addressed by the Quality and Safety Committee a/or Safety Committee

  • Track incidents, present incident reports to patient safety committee and file all mandated safety reports

  • Assists departments with the development and implementation of hospital tracking system for quality management issues.  Orients personnel to required use

  • Assists departments with the integration of quality and risk management activities as the sole contact for risk reporting

  • Reviews departmental quality indicators for compliance, appropriateness and rationale

  • Reviews and analyzes incident reports, rates by severity and conducts all follow-up as identified from review. Completes trending analysis to identify top priority areas for improvement

  • Independently conducts root cause analysis in the event of a sentinel or near miss event. Coordinates review of medical information, debriefs all individuals as identified, performs cause and effect analysis and coordinates and participates in expert panel review, root solution and follow-up for all sentinel events

  • Conducts FMEA analysis when high-volume, high-risk opportunities as discovered through incident report analysis. Organizes team, creates process maps, creates failure modes analysis, identifies prevention and/or corrective action strategies and implants as identified

  • Reports to all major regulatory bodies sentinel events with mandated reporting. Complete autonomy short of proofread by Quality director and CMO

  • Assess incident reports to determine what measures, related to national and local patient safety initiatives, need to be implemented to address concerns.  Partner with the clinical team to create and implement solutions/programs to resolve issues.  Follow through to ensure successful implementation.

  • Acts as liaison to statewide and national collaborative for quality and safety initiatives

REQUIRED EDUCATION AND EXPERIENCE:

  • Bachelor’s degree and three to five years' experience within a healthcare setting with a focus on patient safety, risk management or quality improvement; or equivalent combination of education and experience.

  • Training in patient safety or risk management and quality/performance

  • Experience and/or demonstrated understanding of root cause analysis and FMEA analysis.

  • Experience in statistical analysis tools.

  • Degree in Nursing, Pharmacy, or similar/related clinical field is highly preferred.

  • Experience working in a large and complex health care organization, or relevant clinical

  • Recent experience with core measure data collection and analysis.

REQUIRED CERTIFICATIONS, LICENSES, REGISTRATIONS

  • BLS certification

PREFERRED CERTIFICATIONS, LICENSES, REGISTRATIONS:

  • Current license to practice as an RN in any state is preferred

  • ASHRM(American Society for Health Care Risk Management) or equivalent certification

  • Membership in a professional organization(s)
