As the Hospice Regional QAPI Coordinator, you will be accountable for management and coordination of an integrated Quality Assessment and Performance Improvement program in the assigned region. You will be responsible for the on-going preparation for certification and licensure surveys for LHC Group Hospice locations within the region.
Worked hours may vary with after-work hours and travel. Must be willing to travel 50% - 60%, on a scheduled and emergency basis as needed, to different Hospice locations.

Primary Responsibilities:
- Lead Quality Assessment and Performance Improvement (QAPI) initiatives across assigned Hospice Agencies, focusing on clinical outcomes, internal processes, and family satisfaction
- Ensure regulatory compliance by monitoring state and federal guidelines, preparing agencies for surveys, and coordinating staff education on QAPI and regulatory standards
- Serve as a resource to hospice staff and administrators, supporting efficient operations and quality patient outcomes through data review and corrective action planning
- Conduct on-site compliance surveys and assist with reporting requirements such as action plans, executive summaries, and survey responses
- Support Joint Commission activities and collaborate on developing educational materials and in-service training based on QAPI findings

Required Qualifications:
- Bachelor's Degree in a health care related field OR Associate's Degree acceptable for RN candidates only
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
- 5+ years of health care experience
- Demonstrated understanding of CMS Conditions of Participation, Joint Commission Standards and state licensing standards
- Computer literacy, especially in Hospice Patient Management Systems, Excel, Word, and PowerPoint
- Demonstrated ability to work flexible hours as required to meet regional needs
- Proven effective presentation and communication skills; ability to present information in a clear and concise manner to large groups of people
Preferred Qualifications:
- Master's degree
- Certification in HealthCare Quality
- Hospice experience
