What You'll Do
  • Serve as a seasoned social work consultant in long-term care facilities
  • Provide individual and group psychotherapy sessions
  • Complete mood and symptom-based screenings
  • Develop individualized treatment plans for adults and/or seniors
  • Provide in-service education
  • Collaborate with facility staff and consult with interdisciplinary teams
  • Maintain accurate, compliant documentation

 Qualifications
  • Master's Degree in Social Work
  • Licensed Mental Health Counselor with the ability to practice and bill independently
  • Active license to practice in State(s) where MediTelecare operates
  • Significant experience in treating adult and/or senior patient population, particularly with patients exhibiting severe persistent mental illness
  • Strong communication skills and cultural humility
