Overview:
We are looking for an innovative, passionate person with a true calling for patient care—someone who uses their heart, soul, and mind in creative ways—to join our team as an Exercise Physiologist for our online Cardiac and Pulmonary Rehabilitation program.
In this role, you will plan and deliver engaging telemedicine-based exercise sessions for patients with chronic cardiac and respiratory conditions—including congestive heart failure and COPD—as well as those with multiple comorbidities. We serve a medically complex patient population—typically older adults with multiple comorbidities—who may experience frequent hospitalizations and face psychosocial challenges. Success in this role requires strong expertise in safely supporting special populations with a patient-centered, encouraging approach that helps individuals build confidence and make meaningful lifestyle changes.

Key Responsibilities:
Lead multiple daily telehealth-based group exercise sessions (15–30 cardiac and pulmonary patients), exercising alongside participants to demonstrate proper technique and body mechanics; provide individualized modifications for special considerations and ensure safety at all times.
Monitor vital signs and RPE/RPD for signs of distress or exercise intolerance; use clinical judgment to modify, pause, or hold exercise based on symptoms and medical history.
Independently assess, triage and escalate acute concerns to appropriate providers as needed.
Provide individual and group patient education on disease management, medication regimens, preventive care, smoking cessation, and many other topics
Collaborate with the multidisciplinary care team to coordinate patient care, maintaining regular communication with primary care providers, specialists, and external agencies.
Advocating for patients who are experiencing major social stressors by identifying the need for community resources to improve access to medications, transportation, healthy nutrition, and safe housing
Serving as a strong point of contact for patients and families, providing education, being an advocate and helping them develop self help skills.
Qualifications:
Education & Certifications:
Bachelor’s degree in exercise physiology, exercise science, kinesiology, or related field
Certified in Basic Life Support
Minimum of a Bachelor’s degree in Exercise Physiology, Kinesiology, or a related field.
ACSM Certified Clinical Exercise Physiologist (ACSM-CEP) required, or ability to obtain ACSM-CEP credential within 1 year of hire.
Possession of any additional licenses or certifications required for telemedicine practice in your state or region is preferred.
Group Fitness Instructor Certification a strong plus and preferred
Maintenance of certification and continuing education credits throughout employment
Experience:
At least 2+ years of clinical experience in cardiac and/or pulmonary rehabilitation, with hands-on experience working with high-risk, medically complex populations.
Experience leading group exercise sessions or classes with diverse populations
Prior involvement in telemedicine or digital health programs is a strong plus.
Technical & Interpersonal Skills:
Proficient in using telemedicine platforms and comfortable with digital tools for patient management and education.
Excellent communication skills, both written and verbal, with an emphasis on patient education and team collaboration.
Strong analytical and problem-solving skills, with an ability to leverage data and feedback to drive improvements in patients’ health.
Adept at coordinating care across various disciplines, with a keen focus on delivering high-quality, individualized patient outcomes.
A passion for continuous learning and an innovative mindset to meet the evolving needs of patient populations.
