You will play a vital role in ensuring our members receive the right medications at the right time. By focusing on medication adherence and chronic disease education, you will drive better clinical outcomes for our neighbors and ‘ohana. You will also serve as a bridge between members and their providers, navigating the complexities of medication therapy management with empathy and expertise.
Required Skills & Experience
Local Residency: Must currently reside in the State of Hawaiʻi.
Licensure: Active Pharmacist License in the State of Hawaiʻi, in good standing, at the time of application.
Clinical Experience: 3+ years of clinical experience in direct patient care.
Communication: Ability to communicate complex medical information with a "talk story" warmth that builds trust with local patients.
Desired Skills & Experience
Experience working with collaborative pharmacy practice agreements.
Board certification or specialized experience in complex geriatric care (Kupuna care).
Multilingual skills (e.g., Tagalog, Ilocano, Cantonese, or Hawaiian) are a significant plus.
Your Responsibilities & Impact
Pharmacist Consult Services
Work directly with patients to manage chronic conditions through personalized care plans.
Develop and provide educational materials tailored to the specific needs of our local population.
Collaborate with interdisciplinary teams to iterate on disease management processes.
Help members and providers navigate their benefits to ensure healthcare is accessible and affordable.
Medication Therapy Management (MTM)
Conduct comprehensive medication reviews for members with complex regimens.
Identify and resolve drug therapy issues to optimize care and improve health outcomes.
Quality & Compassionate Care
Drive "5-Star" performance on pharmacy-related clinical measures through best-in-class care.
Address therapy gaps (e.g., statin initiation for diabetes or cardiovascular health).
Interact with members with genuine compassion, helping them reach their individual health goals.
Practice Operations
Support on-call rotations for urgent pharmacy needs.
Act as a clinical subject matter expert for our pharmacy model development.
Mentor and train the wider team on pharmacotherapy and practice-related topics.
Salary Range: $74,000 - $150,000 / year

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
