GENERAL SUMMARY:
In collaboration with the supervising Physician, the Advanced Practice Clinician (APC) will have the opportunity to provide direct patient care in accordance with established policies, procedures and protocols of the healthcare organization.
The APC provides direct, comprehensive medical care to individuals across a broad spectrum of age and development within a high-risk population. Responsible for the diagnosis and treatment/management of medical conditions. Coordinates with community partners to help ensure comprehensive, integrated care.

ESSENTIAL FUNCTIONS:

  • Perform initial and/or periodic histories and physical examinations.
  • Provide and coordinate comprehensive care for patients in the ambulatory care setting and in accordance with training and education.
  • Manage acute, episodic, and chronic conditions and refer patients when disease process exceeds the APC’s education, training, and/or experience.
  • Screen patients to determine the need for further healthcare.
  • Order and interpret diagnostic studies such as laboratory tests, radiological exams, electrocardiograms, or other studies as appropriate and specified in each APC’s clinical privileges.
  • Carry out patient-centered health promotion and disease prevention activities including education and shared decision-making.
  • Provide appropriate periodic mental health assessment, screening, counseling and treatment for mental illness, family violence, and diseases of addiction.
  • Obtain specimens for laboratory testing as needed.
  • Initiate and expedite requests for consultations and arrange special tests and studies.
  • Record progress notes in the patient’s medical record according to the Subjective, Objective, Assessment, Plan (SOAP) format in the patient's chart and maintain longitudinal items (histories, problem list, medication list, etc.) in the medical record.
  • Obtain informed consent and document performed procedures.
  • Educate and counsel patients and families in preventive care, medical conditions, and the use of prescribed treatments and drugs.
  • Prescribe and dispense medications and durable medical devices and supplies.
  • Prescribe and dispense controlled substances within jurisdiction of the APC’s State license and facility policies.
  • Perform excisions, biopsies, incision and drainage, laceration repairs, and additional procedures in accordance with training and clinical privileges.
  • Provide medical and emergency interventions appropriate to the patient’s needs.
  • Manage an assigned panel of patients.
  • Participate in On-Call Duties.
  • May refer complex and high priority cases to supervising/collaborating physician(s) with regards to complicated diagnostic problems, serious illness, complicated therapeutic problems, and re-evaluation of chronic conditions.
  • Contribute to quality improvement or process improvement as necessary.
  • Seek professional growth and development through participation in educational programs, current literature, meetings, and workshops.
  • Perform miscellaneous job-related duties as assigned.
  • Responsible for adhering to all Hutchinson Regional Healthcare System Policies and Procedures.
  • Responsible for routine care and cleanliness of workspace.
  • Participate in evaluation of peers’ records and quality standards according to protocols and receives and implements constructive directives.
  • Maintains collegiality with all members of the inter-disciplinary team.

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

MINIMUM EDUCATION, EXPERIENCE AND CERTIFICATION REQUIRED:

  • Active BLS/CPR
  • PA/APRN licensed to practice in the state of Kansas

PREFERRED EXPERIENCE:

  • 3-5 years of previous clinical work experience preferred
