Teams at Trinity Health (HQ Michigan)

Provide professional nursing care including telephone and walk-in triage, post-discharge follow-up, documentation, order entry, medication reconciliation, patient education, care coordination, prior authorization support, and collaboration with providers and interdisciplinary teams. May direct LPN/MA staff and participate in process improvement, meetings, and cross-site support.

Provides front-desk patient services including greeting patients, registering and verifying insurance, scheduling and confirming appointments, answering phones, managing EMR/charting and payments, supporting patient outreach and intake (including refugee populations and digital campaign follow-up), and maintaining office workflow and confidentiality.
