Provide care coordination and aftercare planning for residential clients in a detox program: assessments, referrals, discharge planning, follow-up, transportation, documentation, group facilitation, and collaboration with clinical team and community providers to meet treatment goals.

Provide initial assessments and care coordination for individuals seeking substance use and behavioral health services. Triage clients with clinical team, determine eligibility, make referrals, maintain accurate EMR documentation, and collaborate with community partners and internal providers to connect clients with appropriate resources.

Provide direct support to clients with substance use disorders in a 24-hour acute treatment setting: monitor detox/recovery, assist with admissions/discharges, perform intakes and documentation, conduct safety checks and searches, facilitate recovery-focused groups, de-escalate crises, and support daily client needs while maintaining therapeutic boundaries and collaborating with interdisciplinary staff.
