Own the success, retention, and growth of a portfolio of health plan clients. Manage senior relationships, lead business reviews, monitor performance, identify risks, resolve operational issues with cross-functional teams, analyze client data, and execute account expansion opportunities.

Provide part-time, remote telemedicine care for older adults: conduct virtual assessments, identify health-related social needs, create care plans for Community Care Advocates, perform asynchronous chart reviews, and help shape clinical workflows and care delivery.

Support pilots and nonstandard referral workflows by executing manual processes, reconciling data, documenting evolving procedures, identifying risks, and partnering cross-functionally to inform automation and SOP development.
