Teams at Old Colony YMCA

Conduct in-reach to correctional facilities to enroll and case-manage returning citizens into education, career, treatment, housing, and community referrals. Create and maintain case files, coordinate services and benefits, maintain monthly contact for 18 months, facilitate mentoring and aftercare groups, collect documentation including urine screens, and collaborate with multidisciplinary teams to support reentry.
