Provides behavioral health care in an outpatient substance use recovery program. Responsibilities include patient assessment, individualized treatment planning, individual, group, and family counseling, crisis response, care coordination, clinical documentation, interdisciplinary collaboration, and quality improvement. Uses evidence-based approaches such as Motivational Interviewing, Cognitive Behavioral Therapy, and Stages of Change to support patients with substance use disorders and co-occurring mental health conditions.

Support quality, population health, and performance improvement by extracting, validating, analyzing, and reporting clinical and operational data from Epic. Build and maintain dashboards, support UDS and regulatory reporting, track quality metrics (HEDIS, PCMH), perform data validation and audits, and collaborate with clinical and operational teams to drive improvements.

Care may be delivered in the PACE Day Health Center or in participant homes.
