Build relationships with ASAPs, SCOs, home care agencies, providers, community organizations, and senior-focused partners. Represent the organization at health fairs and community events, manage partner communications and onboarding, track outreach and referrals in CRM systems, and collaborate with Operations, Clinical, Marketing, and leadership teams to expand healthcare access and organizational growth.
