Embed daily within assigned hospitals to build relationships with discharge planners, nurses, and physicians; educate staff on Abby Care, prospect eligible patients, drive referral volume, streamline hospital-to-home transitions, and rigorously track activities and pipeline in CRM.

The RN Case Manager provides care coordination for patients, develops individualized care plans, maintains clinical documentation, collaborates with healthcare providers, and ensures quality patient care.
