Provide individualized clinical case management for Senior Care Options patients, including comprehensive MDS and PCA assessments, in-home and clinic nursing evaluations, care plan development, coordination with providers and community services, documentation, utilization management, prior authorization assistance, patient education, and participation in interdisciplinary teams and quality improvement activities.

The Pharmacy Intern assists licensed pharmacists in dispensing medications, patient counseling, and inventory management while ensuring compliance with regulations and policies.

The Group Leader supervises school-age children, assists with homework, plans curriculum, and ensures safety. Communication with parents is essential.

The Manager of Workforce Training & Development leads training programs, assesses needs, collaborates with stakeholders, and designs career pathways to enhance workforce skills and recognition.

Provide front-desk and administrative support for WIC clinic operations: greet participants, schedule appointments, screen eligibility, verify documentation, enter participant data, issue WIC benefits, maintain records and inventory, provide referrals, and support clinic workflow and policy compliance.

Provide direct nutrition services for high-risk prenatal, postpartum, infant, and child WIC participants including assessments, WIC food package prescriptions, breastfeeding and nutrition counseling, documentation, referrals, follow-up education, staff training support, quality assurance, outreach, and coordination with healthcare providers across multiple WIC sites.
