Provide comprehensive telehealth assessments for older adults with chronic and complex conditions. Identify clinical risks, care gaps, medication concerns, and recent utilization issues. Oversee individualized care plans with healthcare advocates, address medical and social needs, and collaborate with therapists and care teams. The physician will help shape an emerging care model focused on integrated behavioral and physical healthcare for geriatric patients.

Provides comprehensive telehealth assessments and clinical oversight for older adults with chronic and complex conditions. Identifies medical risks, medication issues, care gaps, and social needs; develops and oversees practical care plans with healthcare advocates; and collaborates with therapists and care teams to integrate physical and behavioral healthcare.
