Firefly Health is a new primary care start-up that launched in 2018 to give more people convenient access to better care. Founded by experienced physician innovators and technology entrepreneurs, Firefly reimagines the traditional care model by connecting patients with a dedicated care team they can easily access through in-app messaging, video, and in-person appointments. 

Because patients work with a dedicated care team, they get to know their providers over time, creating a care journey that is continuous and highly personalized. Care teams learn what matters most to their patients, help them create health and wellness goals, and support them as they pursue them.

Where You Come In:

The Health Coordinator will be responsible for ensuring that the goals of each patient are met in a timely fashion. You will work closely with our team of Physicians and Nurse Practitioners to help patients create and reach their health milestones in a realistic time frame. You will work largely within Firefly’s in-house technology platform to communicate regularly with patients, keep them on track to meet their goals, understand their needs, and solve problems that arise. 

 

What You’ll Do (and much more!) 

  • Longitudinal Care Plans

    • Serves as owner of all patient longitudinal care plans.  Responsible for ensuring that all tasks are followed-up on in timely fashion

    • Communicates with patients to ensure that they are aware and comfortable with tasks related to their goals, and ensures that they complete required tasks on schedule

    • No dropped balls: when tasks are overdue or cannot be completed, escalates to APP, MD, or other team members to review and modify as needed

 

  • Patient relationship management

    • Communicates regularly with patients to ensure that all of their needs are being met

    • Helps patients navigate Firefly application - downloading, using, problem-solving

    • Supports patients’ self management and behavior/lifestyle change  through motivational interviewing, relationship building, engagement, and teaching specific skills (eg, checking blood sugars)

    • Conduct in person and virtual visits with patients to provide guidance and support around desired behavior changes (e.g. weight loss, increased exercise, reduced stress)

 

  • Clinical Care

    • rooms patients, takes vital signs, runs point of care tests during patient visits

 

What You’ll Bring Us:

  • Bachelor’s degree

  • Experience helping people in health care or other setting that requires the following skills:

    • Excellent communication skills, both verbally and written

    • Empathic listening

    • Comfort with diversity, and serving people from a variety of socioeconomic, age, racial backgrounds

    • Communicating and fostering energy and excitement

    • Experience with motivational interviewing a plus; willingness to learn is required

  • Familiarity and comfort with information technology

  • Experience solving problems and being a part of teams building new solutions and workflows

  • Passion for improving health care
